All forms listed below are a required prior to participating in any of the following programs:
HS Fall- Cross Country, Football, Girls Soccer, Girls Swim, Boys Tennis, Volleyball
HS Winter - Basketball, Girls Bowling, Boys Swim, Wrestling
HS Spring - Baseball, Fastpitch, Golf, Boys Soccer, Girls Tennis, Track and Field
MS Fall - Cross Country, Football, Fastpitch
MS Winter I - Boys Basketball, Volleyball

Physical Form- Two-sided, or 2 single-sided pages
Eligibility Questionnaire (Form A) -This is a two-page form, please be sure to print and complete both pages of this form.
Concussion Information- Parent/Guardian and athlete must read and sign each season
Steroid Information- Not part of eligibility process but good information for families.

Safety Guidelines -Sport specific guidelines vary depending on the sport.
Athletic Code Policy- This athletic code is in effect 365 days per year.
Athletic Fee Information Letter 2010-11- For information only. Does not need to be turned in with paperwork.
Reduced Cost Physicals
All student-athletes are required to have a sports physical every two years. For families looking for a place for an inexpensive physical, try contacting the following clinics for more information. The Everett School District does not endorse either clinic and provides the information as a service to families.
